The problem with engineering failures is they keep on happening. There’s nothing new about them; most have happened before and their reasons are well understood.
The difficulty is you arrive late to the party. You’re picking up other peoples’ work; mistakes have already been made in design, purchasing, operation and everywhere else, leaving you the legacy to sort out. You do your best but failures happen, the result of historical reasons and decisions.
Why not kill off failure-inducing rationale and decisions before they cause the problem? You’ll reduce those failures that are well understood but still happen. You’d need rules to work to, and foresight to broaden your viewpoint of where engineering failures are born. There’s lots to consider:
Economics Factory tests
Attitude Design for inspection
Ethics Material choices
Culture Benchmarking
Design life Operator-proofing
Purchasing Equipment ageing
Multiple events Human interactions
And many more.
Let’s shrink them down to the most important 50? Those with superlative general application, squeeze each to a few pages and call them something snappy like …Rules of Thumb? It’s a massive call.
Our 50 rules are drawn from real failures and disasters taking you straight to the principles that are proven failure-causes. Don’t get involved in engineering design, specification, projects or purchasing until you’ve checked out the 50 rules in this book. They could prevent future failures.

NUCLEAR ENGINEERING AND TECHNOLOGY for the 21st CENTURY - MONOGRAPHS SERIES
Jovica Riznic, Series Editor
This monograph discusses the investigated and operational fuel and cladding materials, fuel performance, fuel failure mechanisms and fuel safety criteria. The discussions are limited to widely used UO2 fuel with zirconium alloy cladding, though some materials for the advanced fuel and cladding for existing water cooled and advanced reactors are included to give the reader some preliminary understanding of recent developments in fuel design and material properties. Moreover, monograph was subject to a page limitation, which puts an extra constraint on the coverage of advanced topics. It should be noted that in this monograph, “transient” is used as a generic term to include accidents, since accidents can be considered as fast transients. It also discusses the advanced fuel concept that has been investigated to enhance the fuel performance, i.e., to maintain fuel mechanical integrities and properties required for applicable plant states.

This book offers twenty-four transient heat transfer problems in our everyday lives. Some examples are: How do electromagnetic waves coming from our Sun affect our Earth daily and seasonally? How are ablating materials used to capture most of incoming heat loads to protect a spacecraft, its occupants and its instruments, when entering Earth’s atmosphere at very high speeds? How does frost damage penetrates a spherical and ripe orange when there is a sudden change in outside temperature at night? How much time does it take to roast one piece beef as rare and another same size piece as well done?
Case Studies in Transient Heat Transfer With Sensitivities to Governing Variables will appeal to engineers working in transient heat transfer as well as graduates in mechanical engineering.
Disseminates knowledge by presenting research results, new developments, and encouraging new initiatives in the Oil and Gas industry in India. Areas of impact include Design and Construction, Pipeline Materials, Integrity Management, Health, Safety, and Environment.
